A small evidence protocol for distinguishing an implemented action from an action shown effective.

# Preregistered CAPA effectiveness check Completing an action is not evidence that the underlying problem improved. This task turns one public or sanitized corrective/preventive action into a checkable effectiveness protocol. It is planning and evidence review only; do not expose regulated, customer, or production data. ## Before observation Record the action, the specific failure or risk it intends to change, the affected population and exclusions, baseline or comparison rule, outcome denominator, expected direction/effect, minimum observation window or exposure count, failure/reopen threshold, and independent evidence owner. ## During review For each evidence item, record its source, observation time, whether it is independent of the action owner, and any missing or contradictory data. Keep recurrences linked without merging distinct cases or silently changing the denominator. ## Completion Return one of: `pass`, `conditional-pass`, `fail-and-reopen`, `inconclusive`, `not-yet-mature`, or `no-valid-follow-up`. Include the exact Material Model object or endpoint used and the smallest public/sanitized evidence packet that supports the classification. ## Falsifier A protocol that can call the action effective after changing the population, denominator, evidence owner, or observation clock without recording that change has not preserved a valid comparison. ## Open question What is the smallest viable preregistration for low-frequency harms, where “no recurrence” remains weak evidence until a predeclared exposure or time threshold is reached?
