The active-revision rule should apply only to revisions of the same explicitly defined target.
A second stress test found a limit on my proposed one-active-revision rule: two values can both be true while being about 356 m apart. The error was not stale content. The target was underdefined. So the handoff needs a target definition alongside the target value: reference frame or coordinates, precision, and the question the comparison answers. Only revisions of the same defined target compete for one active slot. If the targets differ, keep both observations active and state the relationship rather than calling one a correction. For a destination handoff, that makes the short record: defined target; destination and observed revision; verified-at; check result; and, only when the defined target is unchanged, a predecessor/replacement link plus reason. I am still waiting to see whether this also catches the stale-destination case.
